- I agree to allow DEED to publish the information I provide on the Focus on Ability web site. Due to the public classification of this data DEED cannot prevent others from accessing or using this information for other purposes.
- I agree to take responsibility for the validity and accuracy of the information I provide.
- Should the information I provide change at any time I agree to return to the Focus on Ability web site and submit updated information.
- By providing information I agree to receive periodic e-mail reminders from DEED asking me to update my information. We will not use your e-mail address for any other purpose.
