To deliver seamless and timely access to the information and help people with disabilities and chronic illness (or their representatives or caregivers) need in order to make information decisions and successfully connect to community resources.
Program Description:
Disability Linkage Line is a free, statewide information, referral and assistance services for people with disabilities, their family members and representatives. Those looking for help can call the toll-free number 1-866-333-2466 during regular business hours. A resource specialist will answer the phone and provide one to one assistance to help assure the caller's needs are heard, and appropriate community referrals are given.
